To download our referral, please click the image to the left it. Once it is downloaded, please print it and take it to your treating GP, Cardiologist, or another medical specialist to complete.

Your Cardiology Appointment

While you wait for your referral letter, please don’t hesitate to contact our offices and arrange your cardiology appointment. Please feel free to download the forms provided below. After downloading them, you can conveniently take them to your referring doctor and kindly ask them to fill them out. Alternatively, you may also ask your doctor to issue a referral to me.

Remember to bring your referral letter when you attend your initial cardiology appointment. My personal assistant will notify you when it is time for renewal.
